In both Safari and Cordova (app), I am seeing this problem: when you plug and unplug headphones on an iPad, sound cuts out permanently. It happens to any sound play, including this demo:

> **[Phaser - Examples - Play Audio File](http://phaser.io/examples/v3/view/audio/web-audio/play-audio-file)**

> Desktop and Mobile HTML5 game framework. A fast, free and fun open source framework for Canvas and WebGL powered browser games.

If you go to that demo on an iPad, play the audio file, then plug and unplug headphones from the iPad jack, audio disappears. Maddening. Anyone figure out a workaround? I’m not sure this is even Phaser-related.
