Forms, Menus, Selects, Etc. No examples?

I’m very new to phaser (but not html5) and I was looking to create a “global settings” scene for my game and a “new game configuration” scene for my game.

I couldn’t find the equivalent of html5’s <select> in phaser-ui, slick-ui, EZGUI, the docs, or any official examples. Also an apparent absence of forms/menus in general.
